The HealthChoices Program is comprised of two (2) components, a behavioral health benefit and a physical health benefit. Individuals can apply for coverage through the County Assistance Office. Most individuals who would be eligible for the Medical Assistance benefit, will be enrolled in the HealthChoices Program, however there are some exceptions such as those in nursing homes. To determine if you are eligible for these services, your Managed Care Organization (MCO), Community Care, will utilize Medical Necessity Criteria to see if you qualify to receive services. In most cases, Community Care will ask your proposed agency to provide information as to why you need these services. Community Care will review your request and make a clinical determination regarding your eligibility.
