The HealthChoices Program is comprised of two (2) components, a behavioral health benefit and a physical health benefit.  Individuals can apply for coverage through the County Assistance Office.  Most individuals who would be eligible for the Medical Assistance benefit, will be enrolled in the HealthChoices Program, however there are some exceptions such as those in nursing homes.  To apply for Medical Assistance and to see if you will be enrolled in the HealthChoices Program, you may apply on line at: https://www.humanservices.state.pa.us/Compass.Web/CMHOM.aspx
